  • Need a PE Waiver Form?

Madison students who are in seventh and eighth grades can elect to waive the state's PE requirement, but need to submit a PE Waiver Form with supporting documentation. (Sixth grade students are not eligible to waive PE at Madison Middle School.) If you are waiving PE because you participate in an organized sport outside of the school day, you need to submit with the waiver a letter from the coach or director of the sport stating the kind of activity and the number of hours the student is involved in the sport. Completed forms should be turned to the student's grade-level counselor.
